f64747b792 bluetooth: shell: Fix includes 
This allows to build the shell with BT_CTLR_DTM and/or BT_CTLR_ADV_EXT
enabled.

The issues has been introduced by commit
bf897cf941 (Bluetooth: Shell: Restructure
shell files).

Moves around the shell files so that they are placed nearer
to the features they expose access to.

A few changes as possible has been made.
