Align the SSP HCI event handlers with the rest of the functions in using
the bt_ prefix. In order to avoid name conflict with the HCI struct
definitions the infix _evt_ has been dropped.

Clarify that the acl_data struct includes the bt_buf type as the
first variable in the user data struct.

-Wimplicit-fallthrough=2 requires a fallthrough comment or a compiler
to tells gcc that this happens intentionally.
