d1a3f4f4fe Bluetooth: Host: Fix monitor UART selection
Most boards set zephyr,bt-mon-uart to point at the same device as
zephyr,console. It's still useful to have the possibility of having these
as two different devices, however it's useful to fall back to the UART
console in case a dedicated monitor UART hasn't been specified.

This also ensures that the fallback only happens if the console UART isn't
enabled, but a DT chosen property exists for it. Additionally, we now get a
user friendly error in case the Bluetooth UART monitor feature has been
enabled in Kconfig but there isn't a suitable UART available for it in
devicetree.

d7da085948 bluetooth: make monitor timestamps roll over less
With e.g. a 216MHz core clock, the 32-bit cycle counter overflows after
just 20 seconds. The 32-bit monitor timestamp (100us resolution) overflows
after around five days regardless of what we do, but we should try our best
to reach that.

ef14c9b867 lib: os: add a header for printk hook functions

a5fd0d184a init: remove the need for a dummy device pointer in SYS_INIT functions
The init infrastructure, found in `init.h`, is currently used by:

- `SYS_INIT`: to call functions before `main`
- `DEVICE_*`: to initialize devices

They are all sorted according to an initialization level + a priority.
`SYS_INIT` calls are really orthogonal to devices, however, the required
function signature requires a `const struct device *dev` as a first
argument. The only reason for that is because the same init machinery is
used by devices, so we have something like:

```c
struct init_entry {
	int (*init)(const struct device *dev);
	/* only set by DEVICE_*, otherwise NULL */
	const struct device *dev;
}
```

As a result, we end up with such weird/ugly pattern:

```c
static int my_init(const struct device *dev)
{
	/* always NULL! add ARG_UNUSED to avoid compiler warning */
	ARG_UNUSED(dev);
	...
}
```

This is really a result of poor internals isolation. This patch proposes
a to make init entries more flexible so that they can accept sytem
initialization calls like this:

```c
static int my_init(void)
{
	...
}
```

This is achieved using a union:

```c
union init_function {
	/* for SYS_INIT, used when init_entry.dev == NULL */
	int (*sys)(void);
	/* for DEVICE*, used when init_entry.dev != NULL */
	int (*dev)(const struct device *dev);
};

struct init_entry {
	/* stores init function (either for SYS_INIT or DEVICE*)
	union init_function init_fn;
	/* stores device pointer for DEVICE*, NULL for SYS_INIT. Allows
	 * to know which union entry to call.
	 */
	const struct device *dev;
}
```

This solution **does not increase ROM usage**, and allows to offer clean
public APIs for both SYS_INIT and DEVICE*. Note that however, init
machinery keeps a coupling with devices.

**NOTE**: This is a breaking change! All `SYS_INIT` functions will need
to be converted to the new signature. See the script offered in the
following commit.

79e6b0e0f6 includes: prefer <zephyr/kernel.h> over <zephyr/zephyr.h>
As of today <zephyr/zephyr.h> is 100% equivalent to <zephyr/kernel.h>.
This patch proposes to then include <zephyr/kernel.h> instead of
<zephyr/zephyr.h> since it is more clear that you are including the
Kernel APIs and (probably) nothing else. <zephyr/zephyr.h> sounds like a
catch-all header that may be confusing. Most applications need to
include a bunch of other things to compile, e.g. driver headers or
subsystem headers like BT, logging, etc.

The idea of a catch-all header in Zephyr is probably not feasible
anyway. Reason is that Zephyr is not a library, like it could be for
example `libpython`. Zephyr provides many utilities nowadays: a kernel,
drivers, subsystems, etc and things will likely grow. A catch-all header
would be massive, difficult to keep up-to-date. It is also likely that
an application will only build a small subset. Note that subsystem-level
headers may use a catch-all approach to make things easier, though.

NOTE: This patch is **NOT** removing the header, just removing its usage
in-tree. I'd advocate for its deprecation (add a #warning on it), but I
understand many people will have concerns.

c44ef044b8 Bluetooth: monitor: Avoid sending corrupted packets over RTT
This fixes sending corrupted (incomplete) packets over RTT. Additional
helper buffer has been added to ensure the complete packet before it
can be sent over RTT. Once the complete packet is collected it is sent
to the RTT control block in non-blocking manner. From now there will
be no situation that only part of the packet (e.g. header) is sent
which lead btmon to fail to parse the corrupted data.

262cc55609 logging: Deprecate v1, default to v2
Reduced logging mode selection to deferred, immediate, minimal and
frontend. Decoupled logging version from mode and created CONFIG_LOG1
which can be used to explicitly select deprecated version.

From now on, chosing CONFIG_LOG_MODE_{IMMEDIATE,DEFERRED} will result
in version2.

Deprecated CONFIG_LOG2_MODE_{IMMEDIATE,DEFERRED} with cmake warning.

Codebase adapted to those changes.

c0808e3f59 logging: Minimal mode configuration cleanup
Remove LOG_MINIMAL kconfig option which was confusing
since LOG_MODE_MINIMAL existed. LOG_MINIMAL was used to
force minimal mode but because of invalid dependencies
it was leading to issues.

Refactored code to use LOG_MODE_MINIMAL everywhere and
renamed LOG_MINIMAL to LOG_DEFAULT_MINIMAL which has impact
on defualt logging mode (which still can be later changed
in conf file or in menuconfig).

d0e0af74da Bluetooth: Add option to log btsnoops over RTT
There is a choice to log btmon logs over UART or RTT.
You can choose number of RTT buffer, set its name and size.

Replaced CONFIG_BT_DEBUG_MONITOR with CONFIG_BT_DEBUG_MONITOR_UART
for UART usage and CONFIG_BT_DEBUG_MONITOR_RTT for RTT.

2716cbcaa6 drivers: susbsys: Check errors on devices selected using dt macro
Some Kconfig defined devices may be defined using dt_chosen_label
function. Since there is no way to ensure a device enabled in dts
is also defined in Kconfig, it may happen that instance is not
actually defined.
In this case device_get_binding might return 0, leading to undefined
behavior in the function that calls it.
When not already done, systematically check return of function
device_get_binding on devices defined through dt_chosen_label macro.
Trigger ASSERT when required and return error when possible.

75f77db432 include: misc: util.h: Rename min/max to MIN/MAX
There are issues using lowercase min and max macros when compiling a C++
application with a third-party toolchain such as GNU ARM Embedded when
using some STL headers i.e. <chrono>.

This is because there are actual C++ functions called min and max
defined in some of the STL headers and these macros interfere with them.
By changing the macros to UPPERCASE, which is consistent with almost all
other pre-processor macros this naming conflict is avoided.

All files that use these macros have been updated.

2975ca0754 Bluetooth: Kconfig: Rename CONFIG_BLUETOOTH_* to CONFIG_BT_*
The API name space for Bluetooth is bt_* and BT_* so it makes sense to
align the Kconfig name space with this. The additional benefit is that
this also makes the names shorter. It is also in line with what Linux
uses for Bluetooth Kconfig entries.

Some Bluetooth-related Networking Kconfig defines are renamed as well
in order to be consistent, such as NET_L2_BLUETOOTH.

a45dac0bd1 dts: fix Kconfig enablment of BLUETOOTH_MONITOR_ON_DEV_NAME
Since we generate BLUETOOTH_MONITOR_ON_DEV_NAME based on dts we need a
!HAS_DTS check around it.  Otherwise we can get into odd build errors.
Its also possibly that we don't specify "zephyr,bt-mon-uart" and in that
case we default to CONFIG_UART_CONSOLE_ON_DEV_NAME (ie 'zephyr,console'
on DTS platforms).

2684775d0f Bluetooth: monitor: Remove interrupt locking from monitor_console_out()
Locking interrupts for a long duration is in general bad design, and
is particularly bad for the controller which depends on low latency
interrupts. Instead of using interrupt locking introduce a new flag to
track the shared buffer usage and simply drop characters if the flag
is set.

789081673f Introduce new sized integer typedefs
This is a start to move away from the C99 {u}int{8,16,32,64}_t types to
Zephyr defined u{8,16,32,64}_t and s{8,16,32,64}_t.  This allows Zephyr
to define the sized types in a consistent manor across all the
architectures we support and not conflict with what various compilers
and libc might do with regards to the C99 types.

We introduce <zephyr/types.h> as part of this and have it include
<stdint.h> for now until we transition all the code away from the C99
types.

We go with u{8,16,32,64}_t and s{8,16,32,64}_t as there are some
existing variables defined u8 & u16 as well as to be consistent with
Zephyr naming conventions.

19d740c162 Bluetooth: monitor: Drop data instead of holding IRQ lock for long
The IRQ lock should never be held for long periods of time. Instead of
using the IRQ lock to prevent monitor protocol corruption simply drop
the data. Follow-up patches will add additional drop count tracking so
that these get reported properly over the protocol.

ac74d8b652 license: Replace Apache boilerplate with SPDX tag
Replace the existing Apache 2.0 boilerplate header with an SPDX tag
throughout the zephyr code tree. This patch was generated via a
script run over the master branch.

Also updated doc/porting/application.rst that had a dependency on
line numbers in a literal include.

Manually updated subsys/logging/sys_log.c that had a malformed
header in the original file.  Also cleanup several cases that already
had a SPDX tag and we either got a duplicate or missed updating.

645f867444 Bluetooth: Take advantage of IS_ENABLED macro for BT_DBG
The new IS_ENABLED macro allows exposing conditionally enabled code
always to the compiler, even though it may not ultimately end up being
built. This is in particular useful for letting the compiler catch any
logging format string errors. Introduce a new BT_DBG_ENABLED macro
that c-files need to define before including <bluetooth/log.h> in
order to choose whether BT_DBG() logs are enabled or not.

When no Bluetooth logs are enabled the patch also modifies the log
macros to have the format strings checked with the help of the
__printf_like annotation and empty static inline functions.

3fcb9c4ec2 Bluetooth: Use _vprintk() instead of _prf()
The Bluetooth thread stack sizes are optimized based on the assumption
that printk is used for logging and not printf. Using _prf() (the
printf backend) risks overflowing the stack, so use the recently
exposed _vprintk instead.

6bc645962a Bluetooth: Move Bluetooth host stack to subsys
Move the Bluetooth host stack from net/bluetooth to
subsys/bluetooth/host. This is preparation for having both host and
controller under the same root, i.e. subsys/bluetooth/.
