Fix incorrect elapsed events value when LLL event prepare_cb
was invoked but was aborted before anchor point sync. This
caused premature supervision timeouts under applications
configured with CONFIG_BT_CTLR_EVENT_OVERHEAD_RESERVE_MAX=n
and CONFIG_BT_CTLR_PERIPHERAL_RESERVE_MAX=n.

Fix missing connection handle invalidate on Controller
power up.
The connection context are zero-initialized on startup and
calls to `ll_connected_get()` would incorrectly return a
valid connection context pointer for connection handle 0.
Relates to commit fa02dc4d02 ("Bluetooth: Controller: Fix
missing reset of connection handle").

When applying the connection parameter update, the window widening
for the previous connection interval was not applied since that is
normally done in LLL
Fixed by applying the window widening in ull_conn_update_parameters()
instead in this case

If the connection handle given to a Read/Write Authenticated Payload
Timeout HCI command is a CIS or BIS handle, the error returned has to
be BT_HCI_ERR_CMD_DISALLOWED with the new TCRL

Several members of the ull_hdr can be removed, saving 12 bytes
from all ULL instances used (and a bit of code as well)
ticks_active_to_start is always 0 and can be removed completely
ticks_prepare_to_start is always set to
HAL_TICKER_US_TO_TICKS(EVENT_OVERHEAD_XTAL_US), so replace usage
of it by this constant
ticks_preempt_to_start is always set to
HAL_TICKER_US_TO_TICKS(EVENT_OVERHEAD_PREEMPT_MIN_US), so replace
usage of it by this constant
In addition, remove logic handling usage of XON_BITMASK since it
was only used by the long removed legacy LL

Fix connection update microsecond interval variable data
type, to use 32-bit so that a value upto 2000 seconds, i.e.
4 seconds interval and 499 peripheral latency can be stored.

A few things were not flagged by CONFIG_BT_TICKER_REMAINDER_SUPPORT
that should have been; This caused building without it to fail
Initialize remainder to 0 where used with ticker_next_slot_get_ext(),
since it may not get set if remainder support is off

Fix BT_CTLR_LOW_LAT_ULL conditional compilation code by
decoupling it from BT_CTLR_LOW_LAT code.
BT_CTLR_LOW_LAT_ULL makes ULL execution context to tailchain
in comparison to continuously process in a while loop.

Fix reset of drift_skip count when an anchor point sync was
missed so that drift compensation is applied on the next
successful anchor point sync. This will improve connection
stability for short overlapping radio event intervals.

The connection event at the connection update instant was
skipped due to previous ticker node in use that is being
stopped was registering a relative occupied time that
overlapped with the new ticker node being started for
scheduling the connection with new interval.
Added mock interface for ticker_stop_abs() hence needed by
the Controller unit testing.

Introduce BT_CTLR_PERIPHERAL_RESERVE_MAX Kconfig option so
that disabling this option will use minimum time reservation
and exercise the peripheral connection event continuation
using is_abort_cb mechanism.

Implement LL_ASSERT_INFO1 and LL_ASSERT_INFO2 for triggering assertions
with parameter information provided for vendor core dump.
Adds Kconfig CONFIG_BT_CTLR_ASSERT_VENDOR by which the new LL asserts
map to BT_ASSERT_VND macros, which shall be implemented in the
debug_vendor_hal.h of the platform. If not enabled, LL_ASSERT_INFO will
map to existing BT_ASSERT_MSG with parameters printed.
Add use of LL_ASSERT_INFO2 where ull_ticker_stop_with_mark() result may
assert.

Generic node_rx_hdr struct contains data not used for done event, so this
pdu specific data is moved to node_rx_pdu to minimize memory usage for
MFIFO_done alloc.
This however calls for a considerable write-through replacing the generic
node_rx_hdr with node_rx_pdu when relevant and needed.

With very fast HCI interfaces (for ex. in the simulated nrf5340)
the tx_rate calculation can do a division by 0
(if to packets are enqueued in the same 32KHz clock period).
Let's avoid it.

In order to have clean, self-contained HCI headers that do not have any
dependencies towards the Host or any other part of the system (except
types), reorganize the headers in the following way:
- Split out the macros and structs from hci.h to a new hci_types.h
- Merge the existing hci_err.h into the new hci_types.h

On connection clean-up release nodes held by LLCP. These are released
in two steps, first on LLCP terminate (in local/remote) by moving
to rx/tx_node_release chains, and then on conn_cleanup_finalize
nodes are released back to appropriate mem-pool/queue.
Rename abort functions to refer to terminate.

When possible re-use the already allocated RX node for notifications.
Store (retain) RX node and Link element on RX if NTF could occur.
Pass link element to LLCP (ull_cp_rx()) together with RX node.
New RX node type RETAIN introduced to signal retention
When no RX node is available allocate one and hold off TX on procedures
until such time that a node is available for NTF.
In case waiting for NTF buffer avail is needed, allocate and store TX
node to use for TX once NTF becomes available.
CIS Established (incl. timeout handling) is now handled entirely as a
specific event driven by ull_conn_iso - ie removal of procedure check
of cis->established and cis->expire, as this is doubling mechanism
in the conn_iso context.
Unit test and helpers updated to handle new node type.
Function ull_cp_release_ntf() was used only in unit test, so moved to
helper context.
Updating release_ntf to handle the fact that with piggy-backing in test
context the node used for NTF can be from two different memory pools

Fix regression in refactored LLCP using uint16_t instead of
uint32_t in storing the win_offset_us value. This caused
connection update to fail with incorrect window offset being
used to schedule the connection radio events.
Regression since commit e1c2c36f56 ("Bluetooth: controller:
llcp: set refactored as default").

Use instant_latency at the CIG start event count to detect
skipped connection event around the instant. In the future
add implementation to compensate for skipped ACL connection
event and adjust the CIS event count at CIG start.

Make Data, ISO, BIS and CIS PDU structure's octet 3 vendor
specific.
This will allow vendors not supporting the octet 3 or
CTEInfo (8 bits) to save 1 octet in their PDU structures.

Legacy Control Procedure fix. Fix missing release of the
node rx allocation that was reserved at the start of the
LE Connection Parameter Request procedure and the procedure
ends with reject indication transmitted to the initiating
peer.

To be able to timely start the first CIS/CIG, if the requested instant
is the next connection event, call ull_peripheral_iso_start directly
from rp_cc_state_wait_rx_cis_ind instead of waiting for next time
through the state machine.
To enable ULL/LLL to prevent adding latency when LLCP is waiting for
instant, local- and remote procedures with instant now expose a function
for checking this state.
Fixed event_counter function to prevent one-off in RX path. Unified to
use single function ull_conn_event_counter.
Fixed LLCP unit tests and added new mock function.
