This change aims to eliminate the dependency on `ctx_shell` in
the Bluetooth `audio/shell/*`, making the code more maintainable.
Replaced `shell_*` functions that depended on `ctx_shell` with
the appropriate `bt_shell_*` functions.

Moves around the shell files so that they are placed nearer
to the features they expose access to.
A few changes as possible has been made.

Adds, removes and modifies includes in all LE audio
files.
Fixes any found spelling mistakes as well.
Fixes a few places where incorrect types were used.

Since the CSIP API expects a set member struct for nearly all
functionality, the reference to the full set member (along with
the CAS specific CSIS) should be given to the application.

Implement the CAP Commander discovery function.
Adds support for it in the shell.
This includes initial babblesim and unit testing as well.
