Remove CONFIG_SDMMC_VOLUME_NAME, and set the disk name based on the
``disk-name`` property. This aligns with other disk drivers, and allows
for multiple instances of the sdmmc_subsys disk driver to be registered.
Add disk-name properties for all in tree definitions for the
sdmmc-subsys disk driver, and change all in tree usage of the disk name

MMC was using SDMMC kconfigs to build disk driver.
This is incorrect, MMC and SDMMC should not be sharing
Kconfigs. Split the drivers/disk/Kconfig.sdmmc into
drivers/disk/Kconfig.mmc and drivers/disk/Kconfig.sdmmc.
Also update disk tests to account for new MMC Kconfigs.

Similarly to what was done in USB or RNG drivers, configure 48MHz domain
clock using device tree.
By default a freq clock check is enabled.

Add stub code for SDIO support, capable of verifying card responds to CMD5.
This commit also changes the architecture of the SDIO probe step to make
adding new protocol support more streamlined, and enable compiling out
support for undesired protocols.

Set default of DISK_DRIVER_SDMMC Kconfig based on the devicetree.
This should avoid having to set the kconfig at the board level
in order to use sdmmc.

use the new SD host controller USDHC driver, so remove legacy NXP disk
USDHC driver.

as for L4/F7 series that have been added before.

Add a Kconfig symbol to enable use of SDMMC hardware flow control.
In specific cases, this feature could help to avoid FIFO ovderrun
and underrun errors.

The files disk_access_usdhc.c, disk_access_spi_sdhc.c,
disk_access_stm32_sdmmc.c, disk_access_ram.c and
disk_access_flash.c are actually drivers for block devices and SD/MMC
controllers. This patch moves this drivers to drivers/disk and
reworks the configuration so that the drivers are selected when
the corresponding node is enabled.
