Fix a memory issue when calling boot_read_bank_header the length should be
the size of `struct mcuboot_img_header`.
Fixes#54459.

The commit switches flash area access from FLASH_AREA_ macros
to FIXED_PARTITION_ macros and to usage of DTS node labels,
to identify partitions, instead of label property.

In order to bring consistency in-tree, migrate all subsystems code to
the new prefix <zephyr/...>. Note that the conversion has been scripted,
refer to zephyrproject-rtos#45388 for more details.
